It can feel like you’re not getting enough air or you can’t take deep breaths.

Nocturnal dyspnea can occur for varying reasons. Most of the time, it’s linked to a medical condition, such as a respiratory or heart condition. It could also be psychological, with conditions like anxiety or trauma.

Read this article to learn why you wake up gasping for air, the most common causes, tests to find out why you’re having this symptom, and when to seek immediate medical help.

Understanding nocturnal dyspnea: why you wake up gasping

Nocturnal dyspnea occurs as a symptom in people with health conditions including anxiety, pulmonary embolism, asthma, pneumonia, heart failure, and chronic obstructive pulmonary disease. It can also present in people with higher weights.

When it happens, you might feel air hunger, like you need to breathe in more air. Your chest may feel tight and your breathing may become rapid. Even then, it may still feel like you’re not getting enough air.

Primary medical causes (sleep apnea, GERD, asthma, heart failure)

Nocturnal dyspnea may happen due to a wide variety of reasons that fall into these categories:

Respiratory disorders

Asthma, bronchiectasis, chronic obstructive pulmonary disease (COPD), pneumonia, and other conditions that affect breathing and the structures that support it, like the upper airway and chest wall, can make breathing difficult or cause you to wake up feeling like you need to catch your breath.

Heart conditions

Conditions that affect how well the heart works can also affect how well oxygen gets delivered through your body, causing shortness of breath. These conditions include arrhythmia, heart failure, and others.

Neuromuscular conditions

Conditions that affect the nerves and muscles and disrupt how they work together can also affect your ability to breathe properly.

Psychogenic disorders

Mental health conditions that lead to physical or bodily symptoms such as anxiety or PTSD can interfere with your breathing.

Systemic causes

Other causes (such as anemia, carbon monoxide poisoning, or renal failure) that affect different parts of your body can also affect your breathing.

Breathing problems while you sleep may also result from a combination of the reasons listed above.

Primary dyspnea

However, most times, when primary dyspnea happens, it’s due to heart failure, asthma, sleep apnea, and acid reflux (GERD), all of which can make you breathe differently when you’re asleep, explains Saema Tahir, MD, a quadruple board-certified physician who specializes in pulmonary disease, critical care medicine, and sleep medicine.

Here’s why, according to Tahir:

“Heart failure lets fluid pool in the lungs when lying flat so people wake gasping.”

“Sleep apnea causes the throat to collapse and cuts off airflow.”

“Asthma tightens the airways most in the early morning hours.”

“Acid reflux sends stomach acid up to irritate or spasm the airway.”

Panic attacks and nocturnal anxiety swallowing air

When you have nocturnal anxiety or panic attacks, it can trigger your body’s fight-or-flight response and increase your respiratory pattern, per Carleara Weiss, PhD, RN, sleep science advisor for Aeroflow Sleep.

“As a result, people start breathing faster,” she says.

Panic attacks and nocturnal anxiety disrupt breathing by triggering sudden awakenings with a racing heart and a smothering or choking feeling that mimics a real emergency, adds Tahir.

“People with panic disorder also tend to breathe more irregularly even while asleep, which may set off these nighttime attacks,” she says.

Sleep paralysis and hallucinatory airway resistance

If you’ve ever felt sleep paralysis, you might have come across the term “hallucinatory airway resistance” in your search for the cause. However, experts say this isn’t a real medical term.

“Hallucinatory airway resistance is not a recognized medical term, but the sensation it describes is real when you have sleep paralysis,” explains Tahir. “In that state, a person wakes unable to move and often feels a crushing chest weight or suffocation.”

The feeling of hallucinatory airway resistance is a brain phenomenon of the sleep-wake transition rather than having an actual blocked airway, says Tahir.

Diagnostic tests: polysomnography and medical evaluation

To understand why you might have trouble breathing when you wake up, you may need to see a doctor for a medical evaluation. They may order tests to identify the underlying medical condition causing your symptoms.

“Diagnostic testing for nocturnal dyspnea is stepwise,” says Tahir. It could start with simple tests and become more complex, if needed.

According to Tahir, first-line tests include:

Bloodwork

A chest X-ray

An electrocardiogram, which records the electrical activity of the heart

A breathing test (spirometry)

Oxygen level tests, which check the oxygen levels in your blood

A BNP (B-type natriuretic peptide) blood test for heart failure

If the cause still remains unclear, she says a doctor might recommend you do these tests:

An echocardiogram, an ultrasound test that takes pictures of your heart

Fuller lung testing to see how well your lungs are working

Chest CT, an imaging test that makes pictures of your chest

A sleep study

Immediate steps and red flag symptoms requiring emergency care

Red flag symptoms requiring emergency care signal a heart, lung, or allergic crisis, says Tahir.

These include:

Sudden severe breathlessness, especially with chest pain or bluish lips or fingertips

Coughing up frothy or bloody mucus/saliva

Fainting

An asthma attack not relieved by an inhaler

Choking with lip or throat swelling

Get immediate medical help if you have any of these symptoms. Also see a doctor if you keep having trouble breathing while asleep or during the day. They can help identify the cause and recommend treatment for your case.

FAQs

What causes waking up gasping for air?

Waking up gasping for air is most commonly caused by medical conditions that disrupt breathing during sleep, such as sleep apnea, heart failure, asthma, or acid reflux (GERD).

It can also be triggered by nocturnal panic attacks or anxiety, which accelerate breathing and induce a choking sensation during sudden awakenings.

How can you tell whether nighttime breathing problems are caused by sleep apnea or acid reflux?

Sleep apnea causes breathing interruptions when the tissues in your throat collapse and physically block your airflow while sleeping.

In contrast, acid reflux occurs when stomach acid travels up the esophagus to irritate or cause spasms in your airway, often leaving a sour taste or heartburn sensation alongside shortness of breath.

When is waking up unable to breathe a medical emergency?

Waking up unable to breathe is an emergency if accompanied by red flag symptoms like sudden severe breathlessness, chest pain, bluish lips or fingertips, fainting, or coughing up frothy or bloody mucus and saliva.

You should also seek immediate care if you experience severe lip or throat swelling or an asthma attack that fails to respond to a rescue inhaler.

If you’ve ever woken up gasping for air, you probably experienced a distressing symptom called nocturnal dyspnea.
